Heading Home

The Heading Home Program is an innovative and exciting new program that targets people with long term, chronic and severe mental illness through assistance with community living. The program incorporates family support and community linkage to ensure best practice and sustainable mental health outcomes.

This service is delivered through consortia with Mind Australia and Eastern Access Community Health (EACH). This group has been funded by Mental Health Branch to provide individualised packages of support to people exiting Secure Units and Community care Units through the Integrated Rehabilitation and Recovery Care Service. This exciting development further enhances ARAFEMI’s capacity to provide focussed, complex community support.
